ESSENTIAL DUTIES & RESPONSIBILITIES:


Under the supervision of the Director of Public Health, the Community Health Worker will
identify and address health disparities within the public health region to support culturally and
linguistically appropriate COVID-19 support services and other social determinants of health related services.

PRIMARY RESPONSIBILITIES:


• Work collaboratively with formal and informal partners and stakeholders across multiple and
diverse community sectors, including nonprofits, health, government, schools, and business.
• Participate in education, training, and technical assistance opportunities.
• Represent the public health region in neighborhood, local, and regional groups, and meetings.
• Facilitate cultural mediation among individuals, communities, and health and social service
systems.
• Provide culturally appropriate health education and information.
• Provide care coordination, case management, and system navigation.
• Provide coaching and social support by advocating for individuals and communities.
• Facilitate direct services to clients with COVID-19 and their family or household members
affected by COVID-19
• Accommodating communication access needs of individuals served through use of qualified
interpreters and translated materials.
• Providing and distributing educational information about COVID-19 vaccinations and general
Department guidance for individual mitigation.
• Creating connections between vulnerable populations and healthcare providers by providing
the following services to vulnerable populations.
• Providing appropriate care coordination, case management, and connections to patient and
family identified community and social services and referrals.
• Assisting with maintaining and/or applying for social services within their community.
• Identifying and helping to mitigate barriers in health care access such as transportation,
language, and childcare.
• Assisting vulnerable populations with navigating the healthcare system.
• Determining eligibility and enrolling vulnerable populations in health insurance plans.
• Providing culturally appropriate health education on topics related to COVID-19, chronic
disease prevention, physical activity, and nutrition.
• Providing informal counseling, health screenings, and referrals.
• Connecting clients with community-based agencies through closed loop and/or warm hand-off
referrals for supports.
• Increasing cultural competence among healthcare providers serving vulnerable populations by
providing services such as educating healthcare providers and stakeholders about community
health needs.
• Managing care and care transitions for vulnerable populations. Advocating for vulnerable
populations or communities to receive services and resources to address health needs.
Collecting data and relaying information to stakeholders to inform programs and policies.
• Building community capacity to address health issues. Ensuring cultural mediation among
vulnerable populations, communities, and health and social service systems serving
vulnerable populations.
• Completing data tracking system forms to document the care coordination and case
management of the patient and family.
• Document encounters upon obtaining the appropriate consent, to identify services, assist in
navigating the healthcare system and support data quality.
• Additional duties as assigned.

EDUCATION, EXPERIENCE, AND SKILLS REQUIRED:


• Significant knowledge of residency or employment within the local community, with
understanding of local culture and values, and familiarity with the resources available within
the community
• Willingness to travel within the public health region (Capital Area, Carroll County, or South
Central)
• Valid driver’s license, reliable transportation, and proof of insurance
• Commitment to working intentionally on diversity, equity, and inclusion across all
responsibilities
• Strong ability to develop and maintain strong, trusted, collaborative relationships with internal
and external stakeholders and colleagues
• Knowledge of federal, state, and local government, nonprofit and public health systems, and
strategies
• Excellent communication and relationship building skills required: including listening skills,
written and oral communication skills
• Considerable experience managing multiple priorities with a high degree of flexibility
• Proficient in Microsoft Office, databases, and web-based applications (internet, email, social
media)

ENVIRONMENT:
The venue of this position is a general office environment that is clean, well-lighted, and
environmentally comfortable, free of fumes or airborne particles, and only minimally exposes
employees to office solvents limited to those relative to copier equipment.
This position requires:
1. Minimal requirement to reach at or above shoulder level; occasional reaching below shoulder
level required
2. Ability to travel throughout the territory required
3. Ability to work a flexible schedule, which may include night and weekends
4. Ability to tolerate prolonged sitting and/or standing and to lift to approximately 25 pounds to
waist high level whenever necessary.
